The investigation of railway accidents provides a valuable opportunity to identify learning that can improve the safety and performance of the railway. These investigations often highlight the need for further research to develop a deeper understanding of the factors contributing to an accident, enabling the industry to introduce effective mitigation measures. Achieving reliable learning requires a systematic understanding of the causal factors that led to the event and how they combined to result in the accident.

Derailments remain one of the most significant railway safety concerns and can occur through a variety of mechanisms. Flange climb derailments are among the most common, while also being some of the most technically complex and challenging to understand.

This case study provides an overview of the processes and evidence used to investigate and determine the immediate causes of a flange climb derailment. It explores the sources of evidence relating to the track, vehicle and operational subsystems involved, and explains how testing and analysis were used to understand the interaction between these elements and identify the factors that led to the derailment.
