Synopsis: In this presentation, Greg describes the Clapham collision of 1988 and the Hatfield derailment of 2000, describing what happened, why it happened and what lessons we need to remember today. Both incidents involved errors, but a number of systemic issues too. And both showed how caution is needed when contemplating change…of any kind.
